If you were previously supported by Queen’s University Disability Services/Accessible Learning Support and are starting a new course, please contact us directly at
accessiblelearning@qub.ac.uk instead of completing a new form.
At the end of this form, you will have the option to upload any documentation that outlines your disability or long-term condition. Supporting documentation is helpful for understanding your needs and identifying reasonable adjustments. It is also required for possible funded support. If you do not have supporting documentation, we encourage you to complete the questionnaire, as you may still be able to access support.
Choosing Your Support Requirements
This form helps us understand any barriers you may experience in your studies and identify support that may help you succeed at university.
Based on the information you provide, Accessible Learning Support will review your responses and any supporting evidence.
In some cases, we may be able to recommend support and create an Individual Student Support Agreement (ISSA) without requiring an appointment.
In other cases, we may invite you to meet with a Disability Officer to discuss your support needs in more detail.
As part of this form, you will be asked to select the adjustments you believe would help reduce barriers to your learning and participation. Your selections will be considered alongside the information you provide and do not automatically mean that all adjustments will be approved.
